As a Substance Use Outreach Clinician with VCH you will :
- Use an outreach model that is strengths-based and inclusive of harm reduction approaches, this position will support client goals including detox / substance use stabilization, treatment and recovery programming, as well as those in the pre-contemplative stage, in addition to supporting wellness initiatives.
- Provide harm reduction education, group facilitation across the transtheoretical model of change, and case management toward and between treatment initiatives are part of the role.
- Establish and maintains relationships with clients.
- Collaborates with multidisciplinary team and other community resources, liaises and consults with internal / external physicians, governmental and community social service, health and mental health agencies to facilitate clients’ wellness journeys.
- Participates in committees and meetings, engages in on-going team planning, development and quality improvement activities, and maintains and updates own knowledge.
- Liaise and consult with internal / external physicians, governmental and community social service, health and mental health agencies to facilitate clients’ wellness journeys.
- Provides direct clinical services in the form of immediate assessment, intervention, referral and support to clients in urgent situations.
- Provides crisis intervention-stabilization as required and collaborates with colleagues and other caregivers in the identification and resolution of client care issues, ethical dilemmas, coordination and integration of care.
- Acts as client advocate, supports client’ recovery goals including building recovery capital, develops, coordinates and delivers evidence based treatment / interventions.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes. Successful applicants may be required to complete a Criminal Records Review Check.
