Job Summary
The Regional Central Intake Director is responsible for overseeing and managing the daily intake process for skilled nursing facilities (SNFs) across a designated region. This role ensures timely response to all incoming referrals, coordinates communication between clinical teams and facility leadership, and manages the referral entry process utilizing the Green/Yellow/Red guidelines. The ideal candidate will have in-depth knowledge of the SNF admission process, strong communication skills, and the ability to multi-task in a fast-paced environment. This position interfaces closely with the admission team in the facility as well as the Regional Director of Business Development. The Regional Central Intake Director will represent all facilities and must maintain a stellar level of professionalism, customer service and empathy in all patient communications.
ESSENTIAL FUNCTIONS
Follow the Green/Yellow/Red guidelines for responding to all referrals through electronic portals (EPIC, AIDA, WellSky, Careport, e-fax) within a goal of 30 minutes or less.
Perform medication cost-outs for necessary referrals to ensure accurate financial assessment.
Communicate "yellow" referrals to the facility's clinical team, including the District Director of Clinical, Regional Director of Clinical, Area Director of Admissions, and facility Administrator for further evaluation.
Enter all referrals into the PCC CRM system accurately for designated facilities.
Collaborate closely with the facility clinical team to ensure smooth transitions from referral to admission.
Communicate effectively and promptly with hospital personnel regarding acceptances and denials
Maintain up-to-date knowledge of SNF admission regulations and industry best practices
KNOWLEDGE, SKILLS, AND ABILITIES
Strong knowledge of the SNF admission process and referral management.
Must understand medical terminology and clinical capability grids (Green/Yellow/Red)
Approachable, flexible and adaptable to change
Organized and detail oriented
Effective verbal and written English communication skills
Familiar with various insurances, including, but not limited to: Medicare, Medicaid, Managed Care
Ability to work with minimal supervision, take initiative and make independent decisions
Comfortable reviewing and responding to SNF referrals through various electronic portals and e-fax.
Proficient with computer systems, including Microsoft Word, Outlook, and PCC.
Strong communication skills, both verbal and written, with the ability to communicate effectively across clinical and administrative teams.
Ability to multi-task in a fast-paced environment, prioritizing tasks based on urgency.
Self-starter with excellent time management and problem-solving skill
EDUCATION, LICENSURE, AND EXPERIENCE
Minimum of 3-5 years of experience in admissions, marketing, or a related field within the Skilled Nursing Facility (SNF) industry.
Clinical licensure preferred but not required
REPORTING RELATIONSHIPS
Reports to Regional Director of Business Development.
