Job Summary
Manages the day-to-day healthcare operations of the community to ensure resident’s healthcare needs are met. Ensures residents are treated with respect and dignity and ensures quality care as resident’s healthcare needs change. Supervises licensed nurses and other direct care staff within the community.
Essential Functions
1. Responsible for the direct supervision of community-based licensed nursing staff (LPN/LVN, RN), and/or CNAs and Personal Assistants.
2. Assigns and directs work of subordinates; appraising performance; rewarding and disciplining associates; addressing complaints and resolving problems. In conjunction with regional operations, develops annual operating and capital budgets. Aggressively anticipates and minimizes negative budget variances and deficits.
3. Provides training, supervision, and monitoring of associates in the administration of medications as described/allowed in Nurse Practice Act, to include auditing of medication administration records.
4. Provides training and education to resident care associates on an ongoing basis with classroom in-services, and situation-specific training.
5. Supervises the maintenance of resident charts and reviews documentation performed by care giving staff.
6. As described and allowed in the Nurse Practice Act, assesses health, functional and psycho-social status of residents, initiates individualized service plans, proactively manages care and services for each resident, evaluates effectiveness and maximizes the resident's opportunity to remain in their environment.
7. Manages the health care of residents, including the dissemination of information to families and associates. Ensures that family members are aware of resident's need for scheduled appointments.
8. Participates in pre-admission screening of prospective new residents. Assures that required documentation is completed prior to or upon resident admission, including nursing assessments, service plans, and other assigned forms. Updates assessments as required by policy and as described/allowed in Nurse Practice Act.
9. Performs on-site evaluations of residents admitted to alternate care environments for treatment, and maintains contact with resident families with the intent of returning resident to Vantage Pointe Village.
10. Performs ongoing assessment/observation of residents' physical and psycho-social needs and coordinates with other departments to assure quality, proactive care.
11. Evaluates residents; documents change in condition, and notifies executive director, physician, and resident’s legally responsible party/family of resident's condition and reactions.
12. Communicates with physicians regarding residents' status and condition changes. Ensures residents are prepared for physician appointments, prepares Physician Visit Form and reviews and updates resident chart.
13. Facilitates continuity of care for those residents receiving home health care, hospice services, and other third party healthcare- related services.
14. Participates in or leads meetings relevant to resident care issues, such as Service Plan meetings, involving appropriate parties as needed.
15. Ensures in-house ancillary medical services such as podiatrist, doctor visits, dental visits, psych visits, lab, X-ray, ambulance, etc. are scheduled and followed through.
16. Participates in department quality improvement activities.
17. Assists in keeping the environment safe for associates to reduce the occurrence of Workers’ Compensation claims by appropriately evaluating resident transfer and mobility needs and involving therapy services as indicated.
18. Shares on-call and manager on duty responsibilities/shifts as required.

Education and Experience
Education as required to obtain state nursing license (LPN/LVN or RN) and a minimum of 5 years relevant experience. Basic typing skills along with a working knowledge of personal computing and word processing software are required, preferably in a Microsoft Windows environment.

Primary Physical Requirements:
1. Lift up to 10 lbs: Frequently required to lift medical charts, supplies, and residents’ personal items, i.e., clothing and food items.
2. Lift 11 to 25 lbs: Occasionally may be required when dealing with boxes, files or medical equipment.
3. Lift 26 to 50lbs: Occasionally required when assisting with resident care.
4. Lift over 50 lbs: Required when transferring a non-ambulatory resident weighing over 100 pounds. Two aids are typically utilized when resident is unusually heavy or combative. Employee must follow specific protocols related to individual resident’s care and transfer procedures.
5. Reach above shoulder height: Occasionally occurs when reaching for medical records, supplies, and linens located on shelves.
6. Reach at shoulder height: May be required when assisting residents in eating, when changing bed linens, when taking vital signs, providing treatments and when assisting with certain activities of daily living.
7. Reach below shoulder height: Occasionally occurs while providing treatments, taking vital signs and/or assisting with activities of daily living.
8. Push/Pull: Frequently required when assisting a resident in a wheelchair: which rolls easily across tile, vinyl or carpeted floor.
Hand Manipulation:
1. Grasping: Frequently grasping while working with files, answering the phone, assisting during resident care and/or completing documentation.
2. Handling: Assisting with residents’ personal items, blood pressure gauge, thermometer, and other equipment related to activities of daily living.
3. Supplies and Equipment: Blood pressure gauge, thermometer, wound care supplies, diabetic supplies, syringes and other nursing care items.
Other Physical Consideration:
1. Twisting: Occasionally may occur while transferring resident.
2. Bending: Frequently occurs while taking vital signs, assisting residents in activities of daily living.
3. Squatting: Occasionally may occur while communicating with a resident in a wheelchair and/or when assisting with certain activities of daily living.
4. Kneeling: Occasionally may occur while assisting with resident care.
5. Stair Climbing: Occasional. Elevators are available.
6. Sit-to-Stand: Frequent.
7. Sitting: Frequent for long periods, occasionally up to 2 to 3 hours at a time.
Work Surface:
Varies from carpeting, vinyl to tile. When seated, typically an office-type chair.
Cognitive and Sensory Requirements:
1. Talking: Necessary for communicating with residents, family members, other aides, and CNA,s, PAs, RAs, Med Techs, and other members of the community. Must be able to speak English fluently.
2. Hearing: Necessary for taking instruction from physicians and requests from residents. Must be able to understand English fluently.
3. Sight: Necessary for doing job correctly and effectively. Must be able to write and read English fluently.
4. Smelling: Smelling is required for accurate maintenance and detection of wounds, urinary tract problems, etc.
5. Reading and writing: Necessary for written communication and following written instructions
