Responsibilities
- Identify and assess patients for needed services, including medical, emotional, and spiritual support.
- Act as a liaison between patients, families, healthcare providers, and support services.
- Ensure compliance with all regulatory requirements and standards of care.
- Provide education and support to patients and families regarding hospice care and services.
- Facilitate communication and collaboration among the multidisciplinary care team.
- Maintain accurate and up-to-date patient records and documentation.
- Conduct regular assessments to identify patient needs and preferences.
- Coordinate and schedule home visits, medical records requests, and other services.
- Provide crisis intervention and support during emergencies.
- Participate in team meetings and case conferences to discuss patient care.
- Provide training and support to hospice staff and volunteers.
- Participate in quality improvement initiatives and projects.
- Manage and resolve patient and family complaints and concerns.
- Stay current with best practices and advancements in hospice care.
Requirements
- Active LVN license
- Minimum of 2 years of experience in hospice or palliative care.
- Strong understanding of hospice care principles and practices.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively with all types of patients and families.
- Ability to adapt as needed to different environments.
- Strong organizational and time management skills.
- Compassionate and empathetic approach to patient care.
- Ability to handle sensitive and confidential information.
- Proficiency in electronic medical records (EMR) systems.

Pay: $60,000.00 - $70,000.00 per year
