The Opportunity
We are seeking a primarily virtual behavioral health clinician to join our dynamic, multidisciplinary team focused on innovating and enhancing programs in primary care. This role involves developing human-centered pathways, refining processes, collecting data, creating educational materials, and supporting Medicaid patients aged 18 to 64 with uncontrolled Type 1 and Type 2 diabetes. You will collaborate on multidisciplinary teams to support complex, high-risk patients, making a direct impact on patient lives.
Key Responsibilities:

Care for a patient panel comprising 50-100 patients living with diabetes and potentially other chronic conditions, collaborating with both remote and primary care team members.

Deliver individual and group behavioral therapy, consultations, community referrals, crisis interventions, and safety assessments for patients.

Engage with patients via the designated technology platform in full compliance with HIPAA regulations.

Conduct behavioral health surveys and assessments with patients as needed to inform and tailor treatment plans.

Gather data and generate reports on metrics pertaining to the behavioral health program.

Participate in design thinking and problem-solving, proposing and implementing new processes and procedures that enhance behavioral health outcomes.

Regularly engage in virtual daily team huddles and weekly case conferences to collaborate, advocate, and discuss patient health needs for the development and enhancement of person-centered action plans.

Facilitate connections to long-term behavioral health services, including referrals to agencies offering supportive services for vulnerable patients, and coordinate arrangements for community behavioral health resources.

Engage proactively in planning and implementing initiatives focused on behavioral health integration, fostering collaboration with external vendors, community-based service providers, and the broader Vayu care team.

Demonstrate organizational adeptness and flexibility in patient care assignments to effectively carry out job responsibilities and meet patient needs.

Act as a mentor and trainer for assigned team members, including developing and presenting training sessions as needed or requested.

Required:

Active and unencumbered license as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC) in the state of California.
- Master’s degree in Social Work, Marriage and Family Therapy, Psychology, Counseling, or a related field.

Minimum of 3 years of experience in a clinical setting, preferably working with patients with chronic illnesses.

Strong clinical skills in assessment, diagnosis, treatment planning, and therapeutic interventions.

Excellent interpersonal and communication skills, with the ability to work effectively as part of a multidisciplinary team.

Proficiency in electronic health record systems and documentation.

Experience educating and advising multidisciplinary care teams on the basic principles of behavioral interventions and reinforcing their use during medical visits.

Willingness to travel for in-person meetings approximately every three months.
