Job Overview:Our client, a reputable Skilled Nursing Facility (SNF) in Auburndale, MA, is looking for a compassionate and skilled Physical Therapist (PT) to join their team. This is a full-time, permanent position with a Monday to Friday schedule (40 hours per week), with weekends as needed. As a PT, you will support the delivery of exceptional physical therapy care to a diverse patient population, including both short-term and long-term care residents.
Key Responsibilities:Deliver direct physical therapy services to patients based on individualized treatment plans.
Assist patients with exercises, activities, and modalities to improve mobility, strength, and overall physical function.
Observe patient progress and communicate any changes in condition or response to treatment to the interdisciplinary team.
Educate patients and families on exercises, adaptive equipment, and strategies for ongoing care at home.
Maintain accurate and up-to-date patient records and document treatment progress in accordance with facility guidelines.
Collaborate with the interdisciplinary healthcare team to ensure comprehensive care for each patient.
Adhere to patient safety protocols and regulatory guidelines during treatment sessions.
Participate in continuous professional development and stay current with industry practices and trends.
Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
Valid PT license in the state of Massachusetts.
Experience in a Skilled Nursing Facility (SNF) is preferred but not required.
Strong knowledge of physical therapy treatment modalities and techniques.
Ability to work effectively with elderly patients, especially those with chronic conditions.
Excellent communication skills, both with patients and the healthcare team.
Ability to work efficiently and manage multiple priorities in a dynamic environment.
Full-time position with a Monday to Friday schedule (40 hours per week).
Weekends may be required based on patient and facility needs.
