What are the responsibilities and job description for the Quality Management (QM) Coordinator position at VEMA Corporation?
The Quality Management (QM) Coordinatorwill be the tasked with training our staff as required
to ensure that everyone stays compliant on their mandatory certifications: CPR, 1st Aid, Prevention & Support, and
Article 9. This position is also responsible for creating, and developing trainings (i.e. leadership training, etc.). As
well as, evaluating the staff’s training progress while providing constructive feedback, so that they may thrive in
their careers or grow into other areas of the business. This is a critical function that requires the individual selected
to be flexible in their hours and open to having changes in their hours. This position is also responsible for taking
on other projects or tasks as assigned.
SUPERVISORY DUTIES/RESPONSIBILITIES
▪ Commitment to the Vision and Mission of VEMA and to be in relationship with community members, participants,
and personnel. Collaborate Executive Director, Operations Administrator, and other personnel as required.
Participate in strategic planning, operational planning, and the budgeting process. Develop and oversee policies
and guidelines for VEMA program, which reflect ethical practices. Develop and execute policies as needed to
increase structure and support strategic direction and mandate of VEMA. Promote the organization to contracted
agencies and maximize public awareness of the VEMA program and attend approved networking functions.
▪ QA is responsible to write, update and implement VEMA, state/local, federal policy and procedures for
Residential Facilities through the Department of Health Residential Licensing and group homes through The
Division of Developmental Disabilities all policies should meet requirements per Article 9 of Health Services,
Article 1 and Article 7.
▪ Responsible for the supervision of all Program Managers, Program Coordinators, and ProgramDirector.
▪ QA must display a commitment and ability to understand and articulate the mission and vision of VEMA
Corporation (Inclusive of becoming familiar and knowing all clients/ residents, case workers, and being able to
multi-task different issues and prioritize issues, projects, fact findings, etc..)
▪ QA is point of contact for Crisis calls after hours. Program Managers and Program Coordinators are to contact QA
for direction, support, follow through of protocol and procedure with incidents.
▪ Responsible for responding to Fact Findings/ Investigations; working with VEMA leadership team to address
concerns and Plan of Corrections
▪ QA makes daily visits to all residential facilities and group homes.
▪ QA will assess and prioritize house checks/ drop-ins daily which can be any shifts either but not limited to;
overnight, evening 4-12am, and weekend visits to all group homes; report is to be given to Executive Program
Director (Report is to be given within 48 hours to EPD) If a plan of correction is needed for care coordination,
house management, etc. QA will follow through if needed and will be staffed with appropriate staff and plan will
be implemented and overseen by Quality Administrator with Program Director.
▪ Develop and maintain a positive and professional relationship with each resident/member and their family or
guardian.
▪ QA should be proficient with the ability to know each resident name, primary diagnosis, case manager, and if
applicable, guardian without always referring to resident face sheets.
▪ Write, complete, and forward all incident and accident reports to case managers, support coordinators, the
Division of Developmental Disabilities and Quality Assurance, or The Department of Health BehavioralHealth.
▪ Corresponds professionally with Network Providers and Case Managers as needed.
▪ Responsible for conducting monthly Quality Management inspections and reports of the findings
▪ Responsible for oversight of completion and development and renewal of all treatment and behavior plans;
setting due dates and addressing changes as needed per client with ProgramDirector.
▪ Assisting Program Director with staffings’, and resident intake/discharge
▪ Responsible for reporting all incidents to the EPD, and RN.
▪ Approving available vacation time of coordinators & managers and implementing policy to ensure coverage is
met to meet the needs of clients. QA will coordinate approved vacation of staff with accounting department as
well as lead a staff meeting to implement coverage plan.
▪ Working with VEMA team and Scheduler to ensure that over-time does not exceed agency budget.
▪ Responsible for abiding by the Code of Ethics, conduct business in a professionalmatter.
▪ Responds to Agency and DDD representatives in a professional and timely fashion.
▪ Completes all reports to Agencies and DDD in a timely fashion.
▪ Completes follow up correspondence via phone calls and emails within 24 to 48 hours to all Agencies, DDD,
Case Managers, residents/members, or responsible parties.
▪ Policy changes, updates, and implementation of VEMA and state and federal regulations.
▪ Bi-weekly census on open beds to contracted agencies.
▪ Participating in referrals sources.
▪ Coordinate admission with Program Director and RN along with presenting VEMA to other agencies for
increased growth.
▪ Attending expansion interviews for DDD.
▪ QA is to complete job responsibilities within a timely manner and meet deadlines efficiently, QA is aware that
position is not limited to what is listed as demand and needs can change.
▪ Ensures all incident reports are completed and submitted to Clinical team within 24 hours.
▪ Ensures safe environment is maintained by communicating procedures with members, support coodinators,
visitors, and agency personnel.
▪ Reports incident on proper forms and informs appropriate staff regarding incidents. Participates in corrective
action and incident mitigation related to incidents.
▪ Responsible for conducting monthly vehicle inspections and maintenance while documenting findings.
▪ Responsible to complete weekly operational schedule: gas; haircuts, work orders, etc.
▪ Provide on-call assistance for member related crisis to managers.
▪ Responsible for abiding by the Code of Ethics, conduct business in a professional matter.
▪ Takes part in developing and implementing individualized service/care plans or person-centered plans for
members and ensures program directives are followed in compliance with State/Federal/local standards.
▪ Participates in staff and administrative meetings as well as ensures home staff meeting are conducted monthly.
▪ Responsible for coordinating fire extinguisher, fire inspections annual renewal with Operations Administrator
and House Managers.
▪ Write and Participate in the development of polices such for infection control, medications, etc.
▪ Conduct continuous improvement of member care to improve clinical outcomes.
▪ Maintain member privacy and stays in compliance with all HIPPA laws.
▪ Provide orientation, supervision and on the job training to new and current employees.
▪ Assists in evaluation of job performance.
▪ Acts as a resource person for paraprofessional staff.
▪ Assists Program Managers with complex medication authorization and refills.
▪ Conducting self-administration and medication training and overseeing Behavioral Health/DDD BHTs, certified care
givers, and direct care staff.

▪ Responsible for verifying all medical forms for each resident are accurate.

POSITION REQUISITES
EDUCATION/EXPERIENCE / KNOWLEDGE/SKILLS:
▪ Has a least a Bachelor of Psychology and/or 5 years of management/supervisory experience in Behavioral Health
Management or related field.
▪ Has at least four years of behavioral health related experience in an assisting living, behavioral health group
and/or residential home, DDD group or nursing home.
▪ Verify and maintain clean Motor Vehicle Driving Record (MVD), current Driver License and own transportation.
▪ Is over the age of 21
