Job Title: Program Manager, Young Adult Supportive Housing
Reports to: Director, Housing Programs
Venture House is a nationally recognized community based non-profit mental health agency, helping adults who are living with mental illness to achieve recovery through improving access to affordable housing, education, healthcare, employment and meaningful social connection. Venture House utilizes a unique approach, replicated internationally, known as the Clubhouse Model. We are seeking an enthusiastic, compassionate leader to oversee and expand a scattered site supportive housing program for young adults living with mental illness in the borough of Queens.
Job Responsibilities:
· Oversee day-to-day operations for the Venture House Queens Young Adult Scattered Site Supportive Housing Program
· Develop partnerships with landlords and building managers to secure apartments
· Establish and maintain network of referral sources for the recruitment of potential tenants
· Collaborate with Venture House Directors in recruiting, training, supervising and evaluating all Housing Program Staff
· Facilitate HRA 2010E applications and ensure an efficient and timely admission and discharge process
· Collaborate with tenants to develop service plan
· Facilitate referrals to community resources: healthcare, benefits and entitlements
· Maintain resident files in compliance with HIPAA standards
· Ensure all periodic apartment visits are conducted as scheduled
· Ensure all documentation requirements are met in a timely manner
· Evaluate all residences for safety and cleanliness
· Maintain repair records
· Act as liaison between residents, superintendents, and building management
· Provide senior level guidance in mediating grievances, utilizing skills of conflict resolution, verbal de-escalation, active and reflective listening
· Document all incident reports in accordance with established protocols
· Maintain required updates in the CAIRS system and track housing data as required
· Oversee case management activities of program staff, including assessment and referrals
· Act as liaison between Venture House and other community stakeholders
· Act as ambassador of the organization with external entities
· Conduct team meetings as appropriate to manage issues and crises
· Ensure adherence to all city, state and federal regulatory requirements
· Prepare reports in advance of program audits
· Participate in quality assurance and quality improvement initiatives
· Manage 24 -hour on-call service
Qualifications: Mental Health Professional with Supervisory Experience Masters/License preferred: LCSW, LMSW, LMHC, LPC, CRC) or substantive experience in the field of Human Services. Valid Driver's License required.
