I. Position Summary
This position description has been prepared to assist in evaluating duties, responsibilities, and skills of the position. It is not intended as a complete list of specific responsibilities and duties, nor is it intended to limit duties to those listed.
This position has responsibilities of a confidential nature. This position works under the general direction of the Director of Nursing with guidance from the Administrator and/or RN Charge Nurse to coordinate resident services in the nursing home. Maintain high standards for customer service in all telephone and face-to-face encounters with both internal and external patients, family members, staff, customers, and physicians while upholding the Vernon County Standards of Excellence.
II. Essential Duties and Responsibilities
· Enters and transcribes admission orders of residents. Ensures room is ready to receive the resident.
· Transcribes provider’s written orders and copies orders onto charts from faxes, transcribing after written, and faxes the pharmacy a copy of pharmaceutical orders.
· Maintains residents’ rights and confidentiality of resident information; reports all suspected cases of resident abuse to the supervisor or Resident Rights Committee.
· Works safely and follows safety practices and standards of the facility; responsible for reporting or correcting any existing or potential safety or accident hazard.
· Employee is expected to report to work when scheduled.
· Assists with admissions of residents by setting up medical records and advising appropriate personnel as needed.
· Assists as needed with discharges.
· Delivers information to units as necessary.
· Maintains medical records by posting progress notes, recording chart orders, charting of weights, BP’s, lab results, etc.,
· Answers telephone. Transfers calls to appropriate staff or takes messages.
· Receives and records call-ins or absences as necessary. Reports these to the appropriate staff for replacement.
· May assist with staff scheduling or replacement at times.
· Responds to and appropriately handles door alarms. Alerts appropriate staff to potential unauthorized exit of residents.
· Coordinate resident appointments, schedule transportation, schedules lab tests, x-rays, etc., as ordered and monitor follow up appointments and testing as needed.
· Prepares paperwork and tubes for lab draws.
· May transport lab work to lab as needed.
· Communicates information from unit to other staff and departments such as nursing, dietary, activity, laundry, housekeeping, therapy, maintenance, social services, transportation driver on admissions and discharges
· May be required to assist with data entry or generating computer reports.
· Must be able to work in an environment where multiple distractions, interruptions, or loud noises are possible.
· May assist with typing or filing, making copies as needed.
· Works closely with Medical Records Department in maintaining charts, etc.
· Responds and participates in disaster and fire drills.
· Assist the Medical Records Department at the discretion of the Director of Nursing or Nursing Home Administrator, and as time allows.
· Assists with delivery mail to residents.
III. Related Job Functions
· Answer phones, may serve as a back up to the receptionist as needed.
· Assist with scheduling of hair care in the beauty shop.
· Reports any unsafe or inappropriate behaviors to supervising nurse.
· May occasionally serve as transportation clerk to take lab specimens, etc. to local health care institutions.
· Keeps unit work station clean and organized.
· Attends in-service programs as scheduled.
· May be asked to assist with residents during emergency circumstances.
· Performs related work as required or directed.
· Knowledge of medical terminology, medications and lab values.
· Ability to read and analyze complex discharge summaries, histories, physical exam notes and handwritten physician’s orders while being alert to possible omissions of information and to carry out sometimes complex written or oral instructions.
· Ability to provide accurate, neat and complete reports and records.
· Ability to enter data with a high degree of accuracy into a computerized program.
· Ability to use computers with proficiency.
· Ability to transfer information accurately and pay attention to detail.
· Ability to communicate, relate and record medical information with accuracy and efficiency.
· Ability to organize and prioritize work.
· Ability to work effectively with other facility employees, residents and guests.

IV. Physical Demands
· A large percentage of time is spent sitting, standing, talking at the central nurse’s station. Uses reading, writing, hearing, talking, near vision, hand and finger dexterity for typing, word processing, calculators, or other office machines. May include some lifting, feeding, or removal of residents from an area deemed unsafe. Must be able to stand for brief intervals, sit for long periods, bend, reach, lift large manuals or move chart racks, move freely throughout the facility, or any other physical requirements needed to perform the day to day functions of this position. Work using concentration in noisy surroundings. Works flexible hours.
· Involves potential and/or direct exposure to blood or body fluids.
V. Education and Training Requirements
· Graduation from an accredited high school or GED. Requires completion of a Health Unit Coordinator course and certification or education and experience as a Medical Records Clerk or nursing student having successfully completed a medical terminology course and medication course. Preference for one year as a Health Unit Clerk or Ward Clerk. CNA certification desirable. Must pass caregiver background check.
· Possession of a valid driver’s license and reliable transportation with adequate auto insurance to meet minimum County requirements, or access to reliable transportation for city and rural travel.
