We’re seeking a dedicated Aftercare Coordinator to lead and coordinate our aftercare services for bereaved families. In this rewarding role, you’ll provide essential grief support, manage aftercare programs, and help families navigate life after loss.
What You’ll Do
As our Aftercare Coordinator, you’ll be responsible for :
- Collaborating with Funeral Directors to assess and meet the needs of bereaved families.
- Preparing and delivering personalized care packets to families.
- Coordinating and executing aftercare programs, including Everything After .
- Maintaining accurate family contact information in Passare and proactively reaching out to offer aftercare services.
- Organizing and managing monthly events (social, educational, or community-based), as well as our annual Service of Remembrance .
- Providing grief care information and referral services to community callers.
- Leading grief support sessions for individuals and groups.
- Presenting to community organizations to raise awareness about available grief care services.
