About Veterans Care Coordination (VCC):

About the Role:
In this hybrid role, you will play a vital role in supporting our veteran and family clients by:
- Building trusting relationships with veterans and their families, listening to their needs, and providing support and resources as needed.
- Coordinating and assisting with a variety of in-home services, such transportation, personal care, and medical supplies.
- Providing referrals to community resources and government benefits programs.
- Maintaining accurate client records and ensuring all information is kept confidential.
- Collaborating with the family and home care agency to ensure client receive holistic support.
Who We Are Looking For:
- Minimum 1 year of experience in customer service, social services, or a related field.
- Strong communication and interpersonal skills, with the ability to build rapport with people from diverse backgrounds.
- Excellent organizational skills and the ability to manage multiple tasks simultaneously.
- Familiarity with veteran resources and benefits is a plus.
- Proficiency in Microsoft Office Suite or Google Workspace is required.
- Strong work ethic, a positive attitude, and a commitment to providing exceptional service.
