What are the responsibilities and job description for the Transitional Housing Case Manager position at Veterans & Community Housing Coalition?
General Responsibilities:
· Ensure resident’s safety at all times
· Greet visitors and/or former residents and ensure compliance with visitation policy
· Provide transportation to and from medical appointments and various activities
· Rotate with Case Management team to provide transportation and/or attend events that occur outside of normal business hours of the agency.
· Professional interaction with residents, while ensuring strong boundaries are in place and exercised at all times.
· Run morning house meeting
· Report to Executive Director any changes to census and program fees. Ensuring Program fees are up to date.
· Maintain records on all residents, including intakes and subsequent service documentation and Progress Notes on a weekly basis, weekly case management meetings with residents to adhere to grant funded requirements. Maintain accurate and up-to-date charting on all residents, including HMIS requirements: progress notes, Quarterly Service Plans, Annual Assessments, monthly Data Quality Reports
· Document and report in HMIS any issues, achievements, regressions, or any information otherwise on resident’s behaviors and/or obstacles hindering their progress
· Ensure residents are connecting with health/recreation programs and services, either private or through the VA
· Act as an advocate for residents to stakeholders and to the resident themselves
· Show support through being attentive to residents’ needs, struggles and achievements
· Communicate with other staff to ensure all needs of the residents are being met
· Network and communicate with similar programs and the VA to advocate for our services
· Assist Guardian House and Foreverly House residents in treatment planning, obtaining federal and local benefits and providing case management services as needed.
· Provide case management services to the children residing at Foreverly House and maintain communication with all education and clinical providers.
· Participate in on-call responsibilities and ensuring access to after-hours emergency care.
· Adhere to the Transitional Housing Training Manual Policy and Procedure’s
· Aid residents in transitioning out of transitional housing and into independent living. This includes but is not limited to, supporting, advocating and talking with veterans., and calibrating with other housing programs (HUD VASH, SSVF, etc)
· Maintain grant reporting and other reports as necessary (i.e. Census, Food Bank, etc.)
· Responsible for all approvals of admissions and discharges of veterans in residential housing. Network and Advocate to maintain consistent referrals and ensure the program is at full capacity.
· Must be able to lift 25 lbs.
· Other tasks and duties as assigned
Job Type: Full-time
Pay: $35,000.00 - $38,000.00 per year
