Job Description:

We are a community-based, not-for-profit hospice & palliative care provider with deep roots in the community, serving all patients and families regardless of their ability to pay or medical complexity.
Responsibilities:
- Assumes responsibility for effective coordination of patient/caregiver care.
- Conducts nursing assessments to assist in overall management of patient/caregiver needs.
- Follows established plan of care, evaluates patient/caregiver responses, and revises plan as necessary.
- Develops, implements, and evaluates plans of care for patients/caregivers.
- Provides interventions consistent with the plan of care and performs all required interventions.
- Maintains liaison relationship with healthcare providers to coordinate multidisciplinary care.
- Identifies and refers patient/caregiver psychosocial needs to VIA Health Partners Medical Social Worker.
Requirements:
- Diploma or Associates Degree in nursing (Bachelors Degree preferred).
- Current, active, and unrestricted compact license as a Registered Nurse (RN).
- Experience in direct care of patients with life-limiting illnesses.
- Prior experience working in pediatric population required for Kids Path Nurse.
