Job Summary:
The Case Worker Supervisor provides oversight, supervision and management of the department to ensure that it meets all VHS, DDS, MRC (ABI}, Transitional Assistance and State regulations and requirements. Additionally, the Case Worker Supervisor provides case management supervision for individuals who are placed in Specialized Home Care, MRC (ABI/TA}, and Transitional Assistance provider homes or living independently within the community. The Case Worker Supervisor will also carry a case load.
Essential Functions of Position:
Clinical and Rehabilitation Skills:
- Conduct monthly meetings with care providers and consumers to monitor ongoing individual support plan goals and consumers' needs
- Meet regularly with Care Providers and consumers in order to provide support in homes Maintain contact with professionals from other agencies to ensure consumers receive appropriate services
- Conduct assessments and work with consumers' ISPs
- Conduct Care Provider home studies and screening of potential placements
- Facilitate placement of new consumers in Specialized Home Care homes
- Evaluate Care Providers' performance in annual evaluations
- Ensures all Care Providers maintain current First Aid and CPR certifications
- Safety Checklist of Care Providers' homes to ensure that homes meet licensing requirements
- Supervise student interns as assigned
- Accompany consumers to medical appointments when clinically indicated
- Supervise assigned Case Workers, respite and support workers
- Assess and maintain contact, where indicated, with clients' biological fam Hies
- Implement strategies to ensure that the home meets the needs and preferences of the individual Teaches daily living skills such as safety rules, travel-training, banking and budget management, cooking, shopping, personal care and hygiene and care of personal space and belongings Encourages clients to utilize developing skills
- Provides supportive counseling to clients concerning daily, personal and social issues
- Assists clients with housekeeping tasks such as cleaning the bathrooms, kitchen, common areas, bathrooms, closets, etc.
- Assists in developing and implementing program activities, which will foster the client's growth, skill development, and healthier lifestyle
Program Management and Operations:
- Assists Senior Management Team to ensure compliance with all internal and external regulatory bodies (QUEST, COA, OSHA and Quality Assurance)
- Maintain ongoing communication with the Director of Services, Service Coordinators and Senior Management Team
- Supervise and perform administrative functions for the ABI/MRC department
- Comply with all corporate and divisional policies and procedures, including safe work practices, worker's compensation, universal precautions, exposure control
- Will be on-call 24/7 for independent clients and Care Providers on caseload
- Evaluate Care Providers' performance in annual evaluation and maintains an annual file
- Conduct monthly record audits to ensure compliance with agency and DDS regulations, policies and procedures
- Perform other duties as assigned by the Director of Services
Professional Development
- Attend and participate in required orientation and learning opportunities that includes, but not limited to, in-service training, conferences and seminars
- Participate in other Professional Development Training as designated by immediate Supervisor or Senior Management
Requirements
Professional Characteristics:
• BA/BS in Human Services or related field
• A minimum of three years' supervisory experience in Human Services, case coordination or counseling/outreach with individuals with developmental disabilities and their families.
Personal Characteristics:
• Excellent written and verbal communication skills
• Ability to work independently and collaboratively with other providers
• A working knowledge of developmental disabilities and therapeutic skills and an ability to relate to a variety of individuals from different cultural and socio-economic backgrounds
Other Qualifications:
• First Aid, CPR and MAP certifications are required, as well as attend meetings on medication administration to gain knowledge and understanding of the medications that clients are taking
• Must possess a reliable vehicle; possess a valid Massachusetts driver's license; meet the company's driver insurance standards; and maintain a safe driving record
• Must have an acceptable CORI (criminal record assessment) and fingerprinting background check
Salary : $55,000 - $65,000
