Description:
Community Health Worker is a public health worker with at least one year of experience working with elders and adults with disabilities who applies their unique understanding of the experience, language, or culture of the populations they serve in order to carry out their responsibilities in AFC-qualified settings.

Requirements:Responsibilities:
- The responsibilities of a community health worker include conducting on-site visits with the member at the qualified setting in accordance with 130 CMR 408.415(B)(6) and (C)(5
- Selecting, training, evaluating, and supervising AFC caregivers
- Assisting with the development of the member’s interim and final AFC plan of care with input from the member or responsible party, all members of the MDT, and other individuals designated by the member.
- Participating in the development, implementation, and ongoing review of the AFC plan of care
- Completing community health worker progress note corresponding with each on-site visit or encounter and upon significant change; or more often as the member’s condition warrants
- Providing to the member and caregiver culturally appropriate health education, information, support, and counseling.
- Reporting changes in the member’s condition to the member’s AFC registered nurse
- Assisting with obtaining information and accessing other health-care and community services. Assisting with making referrals to appropriate service providers if the member requires services other than those provided by the AFC provider
- Reviewing and documenting the fire and safety procedures for the qualified setting
- Participating in discharge and transition planning and implementation
- Coordinates and relays information to VHS staff related to the pre-enrollment process
- Employs lead generation and follow up according to VHS standards
- Maintains accurate, timely and effective records of all leads and application activity in database system
- Maintains ongoing knowledge of changes to Medicare and Medicaid regulations and their impact on daily operations
- Self-reports any potential violations of federal, state or company regulations and guidelines
- Identifies and organizes community outreach activities to spread awareness of coverage option(s)
- Participates in weekly /monthly marketing development meeting
