Brockton Home Health Care agency is looking fora bi-lingual Community Health worker combine with an office manager to performa variety of administrative and clerical tasks. Duties of the AdministrativeAssistant include providing support to assisting in daily office needs,managing our company's general administrative activities also Conduct homevisits to assess living conditions and provide guidance on maintaining ahealthy environment
Community Health Worker
- Conductoutreach and engage with Adult Foster Care and Group Adult Foster Care membersto assess their health needs and provide appropriate resources and support
- Overseeall services provided to individuals and their families, monitor the quality ofcare delivered, ensure compliance with contractual terms, and monitorregulatory program compliance.
- Assistin the development of quality services and engaging activities that meet theindividual served needs.
- Buildand maintain relationships with families and external case managers.
- Providehealth education and promote preventive measures to improve overall communityhealth
- Assistindividuals in navigating the healthcare system and accessing necessaryservices
- Conducthome visits to assess living conditions and provide guidance on maintaining ahealthy environment
- Collectdata and maintain accurate records of client interactions and interventions
- Collaboratewith community organizations to coordinate health-related events and programs
