What are the responsibilities and job description for the Patient Navigator - (Massapequa Park, Hempstead, Hicksville) position at Victory Recovery Partners?
The Victory Patient Navigator assists patients by helping them to navigate the complexities of insurance benefits and financial barriers to care. They provide counseling and guidance to patients to find effective solutions to address identified patient needs. The job location will be the Massapequa Park, Hicksville and Hempstead patient sites.
Job responsibilities include but are not limited to:
· Evaluating and communicating patients’ financial situation, insurance benefits, and potential financial barriers to care.
· Coordinating with patients to help them to compile all information and documents needed for Medicaid eligibility (new applications and renewals).
· Helping patients apply for Medicaid, including following up on submitted applications.
· Working directly with patients to help them understand and navigate the financial complexities of their healthcare, including insurance coverage details, including explanations of out-of-pocket costs (co-insurance, deductibles and out of pocket maximums) and formulating payment plans.
· Identifying and addressing potential obstacles like insurance issues or financial concerns that may hinder access to care.
· Connecting patients with financial assistance programs within the community.
· Maintaining patient records, documenting interactions, and reporting data on patient outcomes and program effectiveness.
· Communicating patient eligibility and benefits.
· Processing payments, issues patient receipts, and maintains collection log of front desk staff.
· Finding effective solutions to address patient financial needs.
· Act as a resource representative when questions arise regarding insurance benefits and account balances.
· Maintaining daily work log of all activities completed.
· Perform other job-related duties as directed by Executive Director.
Qualifications:
· Bachelor’s Degree in a Human Services field
· Medicaid experience required
· Social Services experience required
· Case Management experience required
· Working with the population of individuals with substance use disorder, preferred
· Ability to navigate complex situations and problem-solve effectively
· Detail orientated
· Strong communication and interpersonal skills
Job Type: Full-time
Pay: $25.00 per hour
Expected hours: 40 per week
