Division/Department/Program: Early Treatment Program 

 

View Point Health is seeking a full-time Certified Peer Support Specialists (CPS &) who can join a team of professionals for an exciting program called Project LIGHT Early Treatment Program (ETP) and provide transitional services to clients throughout Gwinnett, DeKalb, & Fulton counties. Early Treatment Program is a team based, multi-element approach to treating transition age youth and young adults, ages 16-30years old with FEP (First Episode Psychosis). This position will be based out of our Norcross Office. The team of professionals will include a Psychiatrist, Registered Nurse, Employment Specialist, Clinical Team Leader, Case Manager, and Certified Peer Support Specialist(s).

CSC (Coordinated Specialty Care) is a collaborative, recovery-oriented approach involving consumers, treatment team members, family members (when appropriate), as active participants. CSC emphasizes shared decision making as a means for addressing the unique needs, preferences, & recovery goals of individuals served. CSC services are also highly coordinated w/ primary medical care, w/ a focus on optimizing consumers overall mental, emotional, & physical health.

 

Duties & Responsibilities:

  • Conduct a minimum of 1 home visit per week/per client within their community setting or within client’s current residence.
  • Assist team members in engaging clients in treatment and keeping clients engaged
  • Provide education to client, family members, and other community partners involved in client’s treatment
  • Attend all interdisciplinary team meetings and provide updates on clients current Strengths, Needs, Abilities, and Preferences.
  • Notifies professionals of any unusual observations or incidents, client progress, and follow up.
  • Provide linkage and assist/coach clients in accessing & obtaining community resources.
  • Write precise, accurate, and clear progress notes in our electronic Care Logic system in a timely manner. Maintain required records and information in each client’s file. Prepares accurate, complete timely documentation (within 24 hours) of case notes and billing information to meet agency and state standards and provide effective service delivery across a broad range of services.
  • Provide individual skills training as indicated in the client’s treatment plan including but not limited to independent living skills, coping skills, symptom management skills, anger management skills, assertiveness skills, advocacy skills, hygiene skills, and many other daily living skills.
  • Recognize the need for crisis intervention and notifies supervisor and professional staff of escalated situations.
  • Meet or exceed Agency Productivity standards 

 

Minimum Qualifications:

Bachelor's degree in a related field from an accredited college or university OR associate degree in a related field from an accredited college or university AND One year of related experience OR HSD/GED AND two years of experience providing social or direct care services to groups of special needs individuals in a human service setting OR One year of experience at the lower level Client Support Worker 2 (SSS011) or position equivalent.

Preferred Qualifications: 

  • 2 years of experience specifically working with adults/youth with severe & persistent mental illness under current CPS certification
 

Requirements/Competencies:

  • Must have a high school diploma
  • Must be eligible for certification as a Certified Peer Specialist- Parent in the state of Georgia or must be a certified peer specialist Parent in the state of Georgia. http://www.gacps.org/Home.html
  • Must have a valid Georgia ID
  • Must perform services within the scope of lived experiences, education and knowledge.
  • Must be able to advocate for client and family and model supportive advocacy
    Must be able/willing to actively seek and manage appropriate care for their family member; and be able to share their own personal story in a safe and appropriate way
  • Must be able to advocate for themselves

  • Must be well grounded in recovery wellness (At least one year between diagnosis and receiving appropriate treatment and/or one year continuous abstinence from substance use.)
  • Provide a letter of reference attesting to the applicant’s commitment to their own wellness and recovery. (Please include contact information)
  • Provide two additional reference contacts (previous supervisors requested)
  • Must be able to lift 20 pounds
  • Requires long periods of sitting/standing
  • Must have valid Georgia driver’s license and Motor Vehicle Record in accordance with company policy
  • Candidates for selection must pass a criminal background check (including fingerprinting)
  • Must pass a pre-employment drug screen and subject to random drug screens
  • Candidates for selection must pass a TB test
  • Any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities Note: Some positions may require a valid driver's license
  • Other duties as assigned
