What You’ll Do (Your Mission)
As a Health Coach (Medical Assistant), you will play a critical role in telephonic patient care, coaching, and engagement. Your mission is to educate, support, and guide patients in managing their chronic conditions, improving their health outcomes, and staying committed to their care plans.
Daily Responsibilities & Key Success Metrics
✔ Outbound Patient Engagement – Make 60 calls per day to ensure patients have an opportunity to connect.
✔ Meaningful Connections – Engage with at least 19 patients per day, delivering quality care management and coaching.
✔ Quality Assurance Excellence – Maintain a 90% quality assurance score to ensure high standards in every interaction.
✔ Inspiring Success Stories – Collect at least 2 patient success stories per month to celebrate and share positive outcomes.
✔Patient Satisfaction – Ensure each assigned patient completes an annual satisfaction survey to help enhance our services.
What You Need to Succeed
✔Certified Medical Assistant (MA)
✔ 3 years of Medical Assistant experience
✔ Bilingual in English & Spanish (Preferred)
✔ Experience with EHRs/EMRs
✔ Strong knowledge of chronic conditions & patient education
✔ Excellent phone communication & motivational coaching skills
✔ Intermediate to advanced computer skills (ability to navigate multiple systems)
✔ Compassionate, proactive, and detail-oriented mindset
✔ Reliable internet connection
