Located in the heart of Greenville, Village Green Rehabilitation and Healthcare Center is a leading skilled nursing facility committed to providing compassionate, high-quality care to our residents. We prioritize the well-being of our residents and staff by fostering a supportive and professional work environment.
We are currently seeking a Director of Social Services to join our team and play a key role in ensuring the psychosocial well-being of our residents.
Job Responsibilities
- Assess the psychosocial needs of residents and develop individualized care plans.
- Act as a liaison between residents, families, and healthcare providers to ensure smooth transitions and optimal care.
- Advocate for residents’ rights and ensure their emotional, psychological, and social needs are met.
- Provide counseling and support to residents and their families regarding adjustment, grief, and other concerns.
- Coordinate discharge planning to facilitate successful transitions back to the community or alternative care settings.
- Supervise and support the Social Services team in meeting regulatory requirements and best practices.
- Maintain compliance with state and federal regulations, including documentation and reporting requirements.
Qualifications
- Bachelor’s or Master’s degree in Social Work (BSW/MSW) or a related field.
- State licensure or certification in Social Work (if required).
- Experience in a healthcare or long-term care setting (preferred).
- Strong communication, organizational, and problem-solving skills.
- Ability to work collaboratively with interdisciplinary teams to enhance resident care.
