VillageCare is looking for a self-motivated and passionate Social Worker Care Manager, who will provide telephonic assessments of member's status and needs. This position works with our patients to develop, implement, and monitor their care plan and ensure continuity of care while monitoring long term care needs. Some daily activities will include :
We would like to speak to those who Minimum formal education of Master's Degree and a current NYS LMSW license. Minimum of one (1) year job-related experience in MLTC. Bilingual skills highly preferred.
