As the Social Worker, you will provide direct services to those MLTC clients / families that require assistance with social, economic, long term planning, counseling and / or motivation fo increased independence.
Key Responsibilities :
- Performing initial psychosocial assessments and
- Follow-up evaluations of members to determine needs, coping skills, barriers to care and social determinants of health
- Responsible for gathering pertinent social information from member's families or agency staff and maneuver conferences, etc.
