Essential Duties and Qualifications
The Resident Navigator supports the multidisciplinary care team by preparing and organizing the team for admitting residents; utilizes effective communication to facilitate a safe and high-quality delivery of resident care. The Resident Navigator reports to the Administrator.
Essential Job Duties and Responsibilities:
- Promote and demonstrate exceptional customer service, including greeting residents and visitors, and providing tours for potential residents.
- Maintains a safe work environment while complying with facility policy, regulations and current infection control practices.
- Initiate and maintain communication with families and/or resident representatives; informing parties of applicable policies and procedures.
- Effectively communicate with pertinent members of the multidisciplinary care team (Admissions, Administration, Social Work, Nursing, etc.).
- Prepare resident rooms for occupancy ensuring equipment is functional, cleanliness standards are met, and the space is absent of environmental concerns.
- Assist in answering and directing phone calls. • Maintain resident business files.
- Appropriately documenting in resident’s medical record; organizing and filing documents in paper and electronic medical records.
- Completes insurance authorizations.
- Other duties as assigned.
Qualifications and Requirements:
- Associate Degree or Equivalent Work Experience.
- Basic computer skills, knowledge of Microsoft Windows and Office.
- Experience maintaining a professional level of patient privacy in accordance with HIPAA procedures.
- Strong written and verbal communication skills.
- Risk of exposure to bloodborne pathogens and hazardous materials, must abide by infection control standards including wearing required PPE.
