Provide Expert Care in Hematology Oncology
As an Advanced Specialty Staff RN, you will play a crucial role in monitoring patients before, during, and after transplantation, addressing their physical, emotional, psychosocial, and financial needs. You will coordinate care across departments, educate patients and families, and serve as a vital resource for the interdisciplinary team.
Responsibilities
- Monitor patient/family before, during, and after transplantation for physical, emotional, psychosocial, and financial needs and intervene when appropriate.
- Refer patients to members of the BMT team (pharmacist, social work, nutritionist, oncology support team, and financial advisors) as needed, and/or utilize community resources when appropriate.
- Coordinate the patient pre-admission work-up and visits.
- Gather and review all pertinent clinical information (including past treatments and outcomes) and present this to the transplant physician.
- Collaborate with direct health care providers to establish plan of care, facilitates continuity of care across departments (both in-patient and out-patient) within hospital and home health providers.
- Coordinate all services for BMT and targeted Blood Cancer patients, and oversee National Marrow Donor Program (NMDP) processes as needed.
- Coordinate the transfer of patient care from Stem Cell Transplant Program back to referring physician and is responsible for long-term follow-up.
- Educate patients, their families, to assist understanding of the disease, and processes and procedures surrounding pre- and post-transplant issues.
- Act as a resource for members of the interdisciplinary team on transplant care and protocols.
- Communicate with interdisciplinary team members, patients and families, and payors in a timely manner to ensure comprehensive documentation.
- Perform admissions and continued stay reviews as needed.
- Act as an advocate for patient's benefits. Manage outpatient drug benefits.
- Keep current on requirements mandated by regulatory bodies.
- Participate in weekly transplant team meetings.
- Enter select patient data into database; reviews information and updates as required.
- Attend meetings and participate on committees as appropriate.
- Graduation from an accredited nursing program.
- Upon hire: Washington Registered Nurse License.
- Upon hire: National Provider BLS - American Heart Association.
- 1 year Hematology Oncology Nursing experience.
- Upon hire: Oncology Certified Nurse (OCN).
- Work at a leading Level I Trauma Center with a renowned oncology program.
- Provide specialized care to patients undergoing complex hematology and oncology treatments.
