Pay: $25 / Hour or DOE - Plus Benefits
POSITION PURPOSE:
The Client Care Manager is responsible for the coordination of services between clients and caregivers for the implementation of schedules, adequate staffing and continuity of care to best meet the needs of the clients. In addition, the Care Manager supervises caregivers and establishes/maintains client and caregiver relationships. Care Managers are the “Face” of Visiting Angels in their community. The Client Manager reports directly to the Operational Manager or Care Director.
This position is Monday-Friday 8:00 AM - 4:30 PM at our Durango office located at 610 8th Ave. Durango CO. Traveling within your designated area to Prospect/Client homes.
PRINCIPAL ACCOUNTABILITIES:
- Demonstrate dependability and integrity in all aspects of the job.
- Maintain timely Documentation of daily events, data entry for schedules, and maintain client care information.
- Timely Documentation and filing of appropriate case information.
- Marketing to appropriate facilities and programs within your community- maintain a good reputation for the business and perform consistent marketing visits throughout the month
- Participate in evaluations of caregiver staff.
- Recognize and reward good job performance and promote caregiver recognition.
- Maintain professional relationships with clients and employees, assuring clients and employees’ confidentiality.
- Maintain positive relationships with all Durango and Cortez Clients and referral sources.
- Respond to and resolve any client or caregiver complaints, incidents or concerns.
- Communicate and refer appropriate matters to the Care Director or Chief Operating Officer.
- Communicate and reinforce Visiting Angels’ policies and procedures, as well as our standard of excellence.
- Fill in as On-call some evenings and weekends, as needed.
- Participate in weekly and monthly team meeting and project to improve our business.
- Answer the telephone and communicate efficiently while upholding Patient Privacy.

REQUIRED
This is a leadership position and as such this position requires that candidates must have previous experience in Home Care, Elder Care, or a similar field - either personal or professional, Long-Term Care Facilities, Medical Facilities, nursing/medical background, etc.
SPECIFIC JOB KNOWLEDGE, SKILLS AND PERSONAL ABILITIES:
- High School Diploma and two years of experience in an office setting, preferably in health care. College degree preferred.
- Demonstrate proficiency with Microsoft Office (Word, Excel and Outlook) applications, scheduling systems and other health care industry related software.
- Ability to listen and communicate clearly, fluently and diplomatically orally and in writing.
- Ability to remain flexible, resilient, calm and maintain a sense of humor; and present a well-groomed professional image.
- Ability to plan, organize, prioritize, delegate and accurately follow through in work activities with time constraints and interruptions to meet deadlines.
- Ability to work independently with a minimum amount of direction and/or supervision.
- Ability to generate goodwill for the organization and its management among staff, clients, and local community referral sources.
- Demonstrate a strong commitment to Excellent client service.
- Possess and maintain good physical and mental health, including current TB testing.
- U.S. Citizen or evidence of valid Alien Work Permit.
