- Patient Care Provide nursing care to hospice patients during after-hours, addressing physical, emotional, and spiritual needs.
- Emergency Response Respond promptly to patient calls, perform on-site visits as needed, and manage urgent patient needs.
- Symptom Management Assess and manage pain and other symptoms, providing comfort and relief to patients.
- Family Support Offer guidance and emotional support to families during challenging times.
- Collaboration Communicate effectively with the interdisciplinary hospice team, including physicians, social workers, and chaplains.
- Documentation Maintain accurate and timely records of patient care and communication.
- Adhere to Best Practices Follow VNA Health Group's hospice best practices, ensuring compassionate and patient-centered care, effective communication, and continuous quality improvement.
- Education Current NJ RN license
- Experience Minimum of 1-2 years of hospice or palliative care experience preferred.
- Skills Strong assessment, critical thinking, and communication skills.
- Availability Ability to work night shifts, including weekends and holidays as scheduled.
- Transportation Reliable vehicle, valid driver’s license, and proof of auto insurance.
