Here’s how you’ll move healthcare forward in this role
- Take part in a two-week clinical orientation to learn key procedures, the EMR system, compassionate care, and safety.
- Assess and evaluate the needs of patients with multiple disease processes and comorbidities and develop an appropriate plan of care
- Provides education and strategies for patients and families
- Coordinates referrals to home care to promote continuity of care
- Partners with other VNA clinicians and external providers to execute best patient outcomes
- Documents all changes in patient’s condition and updates care plans and outcomes
- LPN; Graduate of an NLN accredited LPN program with current NJ LPN licensure
- 2 years of home health nursing experience
- Excellent verbal and written communication skills
- Excellent interpersonal and organizational skills
- Strong client focus, assertive, decision making skills, articulate, and creative in problem solving
- Strong decision making skills
