- You will work closely with our supportive leadership team to provide exceptional care to patients.
- Develop and implement social work plans of care to address psychosocial problems and improve patient outcomes.
- Collaborate with professional staff to identify community resource options and link patients with needed resources.
Responsibilities:
- Participate in a comprehensive clinical orientation program to learn key procedures and safety protocols.
- Provide assessments of social and emotional problems that impact patient care and recovery.
- Adhere to agency policies and procedures on documentation and infection control.
Qualifications:
- Masters Degree in Social Work required, with current NJ LSW licensure.
- 1 year of supervised clinical experience, preferably in community health settings.
- Excellent verbal and written communication skills, with ability to work with diverse populations.
- Bilingual Spanish-speaking skills a plus.
