Summary:
he Hospice RN Flex provides skilled, professional nursing care to hospice patients in their homes, following physician orders and in compliance with the Hospice Medicare Conditions of Participation, Pennsylvania licensure laws, and agency policies. Reporting to the Hospice RN Clinical Manager, the RN Flex plays a crucial role in the interdisciplinary team, responsible for hospice admissions, revisits, and death visits. This position ensures safe and high-quality delivery of nursing services and hospice care, coordinating care to align with the agency’s philosophy and standards. Close collaboration with team members ensures comprehensive patient and family support throughout the end-of-life care process.
Nursing Process
1. Assessing and Planning
- Conduct comprehensive nursing assessments, including physical, emotional, and psychosocial aspects of patients, families, and caregivers.
- Develop and contribute to individualized interdisciplinary plans of care to address palliative comfort measures.
- Consistently collaborate with physicians and the interdisciplinary team to plan and implement care.
- Facilitate and participate in interdisciplinary team conferences to ensure coordinated and responsive care.
2. Implementing and Evaluating
- Provide nursing interventions, preventive measures, and therapeutic nursing procedures as appropriate.
- Collaborate with team members to provide continuous assessment and initiate appropriate therapeutic actions.
- Educate patients, families, and caregivers on disease progression, symptom management, and self-care techniques.
- Document accurate and complete clinical notes and maintain compliance with agency policies and Medicare regulations.
Education
- Mentor team members by assisting with orientation and providing field training.
- Facilitate in-services and participate in patient care conferences to support professional growth.
- Provide education on disease processes and hospice philosophy to patients and families, as appropriate.
Leadership
- Exhibit leadership and critical thinking in care planning and team coordination.
- Function independently, managing time and assignments constructively to meet patient needs effectively.
Communication
- Maintain confidentiality and open communication with patients, families, and team members.
- Actively participate in interdisciplinary team conferences, sharing relevant information and suggestions.
- Exhibit empathy, respect, and professionalism in all interactions.
- Support patient satisfaction and maintain accountability for positive care outcomes.
Resource Utilization
- Coordinate travel time and supply inventory to deliver cost-effective care.
- Participate in on-call services as needed, ensuring clinically competent and responsive coverage.
Secondary Duties
- Act as a backup for the Hospice RN Clinical Manager in their absence.
- Bereavement Program Support as needed.
- Other duties as assigned.
Physical Requirements and Working Conditions
- Must have visual, hearing, and manual dexterity abilities sufficient for effective communication and documentation.
- Able to travel within the service area with a valid Pennsylvania driver’s license and reliable transportation.
Qualifications
- Graduate of an accredited nursing program (bachelor’s preferred).
- One year of general nursing experience, with hospice or palliative care experience preferred.
- Licensed Registered Nurse in Pennsylvania, with CPR certification.
