Position Summary:
As a Hospice RN Flex, you'll deliver skilled nursing care to hospice patients in their homes. You'll play a key role in our interdisciplinary team, handling hospice admissions, revisits, and death visits while ensuring the highest standards of care. This role combines autonomy with teamwork, enabling you to support patients and their families during their end-of-life journey.

This role combines autonomy with teamwork, enabling you to support patients and their families during their end-of-life journey.
What You'll Do:
- Conduct comprehensive nursing assessments and develop individualized care plans.
- Provide therapeutic nursing care, symptom management, and patient education.
- Collaborate with physicians and the interdisciplinary team to ensure coordinated care.
- Educate and mentor team members, caregivers, and families on disease processes and hospice philosophy.
- Participate in on-call services and provide leadership in care planning.
- Maintain accurate and timely clinical documentation.
What We're Looking For:
- Graduate of an accredited nursing program (Bachelor’s degree preferred).
- Current Pennsylvania RN license and CPR certification.
- At least one year of nursing experience (hospice or palliative care preferred).
- Strong critical thinking, communication, and teamwork skills.
- Valid Pennsylvania driver’s license and reliable transportation.
