What You’ll Do:
Deliver high-quality, skilled nursing care to hospice patients, families, and caregivers under the direction of the Hospice RN and Clinical Manager.
Implement and follow personalized care plans to ensure comfort and dignity for patients.
Administer medications and provide pain and symptom management in compliance with state regulations and agency policies.
Provide patient and family education about care techniques and support systems.
Document patient care and condition updates accurately and in a timely manner.
Work collaboratively with the interdisciplinary team to meet the physical, emotional, and spiritual needs of patients and families.
Offer emotional support and assistance with daily living activities to patients and caregivers.
Participate in on-call rotations and weekend visits as needed.
What We’re Looking For:
Education: Graduate of an accredited practical nursing program.
Experience: At least one year of general nursing experience (hospice or community health experience preferred).
Skills: Strong communication, teamwork, and critical thinking abilities.
Licensure/Certifications: Current LPN license in Pennsylvania, valid driver’s license, and reliable transportation with required insurance coverage.
Qualities: Compassion, empathy, and dedication to providing excellent hospice care.
