As a Medical Social Worker, you will play a vital role in delivering psychosocial services, offering emotional support, and helping families navigate the challenges associated with terminal illness.
Key Responsibilities:
Complete comprehensive psychosocial and bereavement risk assessments within five (5) calendar days of patient admission.
Provide supportive counseling, anticipatory grief support, and problem-solving services to patients, families, and caregivers.
Develop and implement individualized care plans in collaboration with the interdisciplinary team.
Assist families with community referrals, financial assistance, and respite care coordination.
Facilitate placement of patients into facilities for respite, inpatient, or basic care when necessary.
Participate actively in interdisciplinary team meetings, staff meetings, and in-service education sessions.
Offer bereavement visits, support groups, and grief counseling as needed.
Ensure compliance with Medicare Hospice Conditions of Participation, agency policies, and local, state, and federal regulations.
Provide education and advocacy regarding hospice care to patients, families, and the community.
Qualifications:
Education:
Master of Social Work (MSW) from an accredited program, or
Bachelor's degree in Social Work from an accredited institution.
Experience: Minimum 1 year of social work experience in a healthcare setting.
Licensure: Licensed Social Worker (LSW) in Pennsylvania (for MSW).
Certifications: CPR certification.
Other Requirements: Valid PA driver’s license, reliable transportation, and auto insurance.
