Job Summary:
We are seeking a compassionate and skilled Medical Social Worker to join our Hospice team. This professional role is responsible for providing a wide range of psychosocial services to terminally ill patients and their families. Duties include conducting comprehensive psychosocial assessments, addressing anticipatory grief, providing counseling, and creating tailored care plans alongside an interdisciplinary team. The ideal candidate will demonstrate empathy, resourcefulness, and the ability to navigate complex care needs in a sensitive and timely manner.

The ideal candidate will demonstrate empathy, resourcefulness, and the ability to navigate complex care needs in a sensitive and timely manner.
Key Responsibilities:
Social Work Services
- Conduct comprehensive psychosocial and bereavement risk assessments within five (5) days of hospice care election.
- Provide ongoing emotional support, counseling, and resources to patients and their families.
- Facilitate referrals to community resources and assist with financial and respite care needs.
- Collaborate with the interdisciplinary team to develop, implement, and revise psychosocial care plans.
- Assist with patient placement in care facilities, as appropriate.
- Participate actively in team meetings, in-services, and support groups.
Resource Utilization
- Manage travel time and caseload effectively while ensuring compliance with hospice reimbursement guidelines.
- Maintain accurate and timely records in accordance with agency standards.
- Participate in evening/weekend on-call rotations as needed.
Customer Service
- Uphold a high standard of customer care by addressing patient and family needs with respect, empathy, and professionalism.
- Demonstrate accountability for patient satisfaction throughout the care process.
Quality Assurance/Performance Improvement (QAPI)
- Identify opportunities for quality improvement and participate in performance monitoring initiatives.
- Support clinical record reviews and contribute to projects and committees.
Secondary Duties
- Provide community education on hospice care through speaking engagements and in-service training.
- Perform additional tasks as assigned by the Director of Hospice.
Qualifications:

Education:
- Master of Social Work (MSW) degree from an accredited school of social work, or
- Bachelor’s degree in social work from an accredited institution.

Experience:
- Minimum of one (1) year of social work experience in a healthcare setting.

Licensure/Certification:
- Licensed to practice social work in Pennsylvania (if MSW).
- CPR certification.
- Valid Pennsylvania driver’s license, reliable transportation, and automobile insurance.
Physical Requirements:
- Ability to work in various environments, including patient homes and facilities.
- Strong verbal communication, stress management, and manual dexterity skills.
