What are the responsibilities and job description for the In Person Nursing - Health Education for Older Adults position at Vital Care Services?
We are looking for an enthusiastic registered nurse to visit an older adult living community location on set 1 day of the week (9am-5pm with one hour lunch) to perform light chronic disease nursing duties. (BP and other vitals screenings fall assessment). Weekdays, 9am-5pm shift.
We need someone available a set a days a week.
For example every Monday from 9am to 5pm.
This is an immediate opportunity starting in the next 7 days.
DUTIES AND RESPONSIBILITIES:
- Collaborates with other members of social services staff and with other health care professionals in an effort to develop individualized care plans for residents.
- Provides information on community resources to assist the physician, patient/family in making appropriate health care plans.
- Assesses patients physical and psycho-social needs in order to design a comprehensive goal-directed plan of care and refers to other team members based on patients’ needs and lifestyle.
- Works collaboratively as part of a multidisciplinary team with other staff
- Demonstrates sound judgement and independent problem-solving skills in order to initiate appropriate interventions with regard to patient's social, psychological, and or physical impairments.
- Provides in-services and educational information to community residents and staff in order to broaden their knowledge of existing resources in the health care field.
- Promotes and maintains health of residents through teaching and counseling.
- Provides health screenings, including blood pressure monitoring in order to maintain and promote good health amongst residents.
- Makes home visits in emergencies and when clients are unable to come to office.
- Serves as a liaison with community agencies to facilitate the process of referrals and ensure linkage with needed community services.
- Maintains standards of practice consistent with the philosophy, objectives and policies.
- Administer and evaluate health indicator surveys as required
- Complies with requirements of any grant-funded programs of the location.
QUALIFICATIONS:
- New York state R.N. license
- Demonstrated organizational skills
- Experience working with seniors preferred
- Knowledge of health issues and concerns of aging
- Understanding and appreciation of role of race, religion, ethnicity, sexual preferences and individual values as it relates to serving clients and their families
- Ability of visit clients at home
- Ability to coordinate care with community health care providers as indicated
- Ability to work independently and also work as a member of a team
