Job Summary:
Vital Medical Associates P.C. is seeking a skilled and compassionate Advanced Nurse Practitioner (ANP) to join our healthcare team. The ANP will provide comprehensive, patient-centered care to home-bound patients, delivering both home-based treatments and in-clinic management under the supervision of our Internal Medicine Doctor. This role involves conducting house calls, assessing patient health, diagnosing and managing chronic conditions, prescribing treatments, and collaborating with multidisciplinary teams to improve patient outcomes.
Key Responsibilities:
- Perform home visits to evaluate, assess, and manage the healthcare needs of home-bound patients.
- Provide primary care, treatment, and management of chronic and acute conditions in both home and clinic settings.
- Work collaboratively with the Internal Medicine doctor to establish, implement, and monitor patient care plans.
- Conduct physical exams, take medical histories, and interpret diagnostic tests to make informed treatment decisions.
- Prescribe medications, therapies, and interventions in compliance with state and federal regulations.
- Educate patients and their families on disease prevention, healthy lifestyles, and treatment options.
- Document all patient interactions, treatments, and changes in condition accurately and promptly in the electronic medical records (EMR) system.
- Provide support in the clinic setting, assisting with patient assessments and managing day-to-day clinical operations as needed.
- Maintain a compassionate, empathetic approach to patient care, building strong relationships with patients and their families.
- Stay current with medical best practices, emerging treatments, and healthcare trends relevant to internal medicine and home care.
Qualifications:
- Master’s degree in Nursing (MSN) from an accredited program.
- Active Nurse Practitioner license in the state of IL..
- Certification as an Advanced Practice Nurse (APRN) in Family Medicine or Adult-Gerontology.
- Minimum of 3 years of experience as an Advanced Nurse Practitioner, with a preference for experience in home care or internal medicine.
- Strong clinical judgment, diagnostic skills, and the ability to work independently.
- Excellent interpersonal and communication skills, with a compassionate, patient-focused approach.
- Ability to work collaboratively within a team environment.
- Valid driver’s license and reliable transportation for home visits.
- Knowledge of EMR systems and proficiency in medical documentation.
Work Environment:
- Primary duties will involve both house calls and in-clinic patient care.
- Flexibility with working hours and travel to patient homes as required.
