The Caseworker plays a pivotal role in patient care operations by providing direct casework services to patients and families within the VITAS Healthcare program.

Key Responsibilities:
- Assist patients and families with community resources utilization, fostering strong relationships through regular patient contact.
- Develop and maintain extensive knowledge of local community resources, including assistance programs at the county, state, and federal levels.
- Work closely with minimal supervision to ensure seamless delivery of casework services.
- Become proficient in company software programs to optimize patient care outcomes.
Qualifications:
- Maintain a minimum of two years' experience in healthcare environments where community resource utilization was a primary function.
- Possess reliable transportation with adequate insurance coverage or demonstrate the ability to cope with the stress of repeated loss.
- Hold a Bachelor's Degree in Social Work or a related Social Science field from an accredited institution.
