POSITION SUMMARY: This position provides continuity of care, with the goal of permanent placement for children through a family care management model that includes developing, expanding, accessing, and linking resources in the community to the needs of children and families. This position will handle out-of-county cases from within Circuit 6 or outside the judicial circuit.

What You Will Do

  • Communicate effectively, both verbally and in writing.
  • Document timely in a clear, concise manner.
  • Assess, interpret, and deliver care-related information for children and families.
  • Work as part of a team managing a caseload of children (birth to 17), primarily handling out-of-county cases.
  • Maintain a positive attitude in a challenging environment, demonstrating patience, respect, and cultural sensitivity.
  • May require the use of a personally owned vehicle.
  • Serve as a mandatory reporter to ensure child safety and protection.
  • Demonstrate knowledge of community resources.
  • Hold a valid Florida driver’s license and be insurable by the agency’s insurance carrier.
  • Arrive on time and be prepared for work.
  • Adhere to EEO compliance and maintain a respectful workplace.
  • Work cooperatively in a team setting.
  • Follow guidance and direction from supervisors.
  • Uphold the agency’s core values and mission.
  • Maintain a flexible work schedule.
  • Proficiency in Microsoft Office Suite.
  • Actively contribute as a member of a family care team.
  • Provide case management services, including assessment, planning, linking, monitoring, advocacy, and case plan assistance.
  • Maintain communication with primary case workers regarding child safety and concerns.
  • Notify primary case manager of critical incidents immediately.
  • Keep supervisors informed of child safety issues requiring attention.
  • Document home visits in FSFN within two business days.
  • Complete home studies for placement as needed.
  • Assist families with case management and provide educational resources.
  • Maintain accurate and timely electronic and paper records.
  • Refer children and families to appropriate community services.
  • Treat children and families with respect and courtesy.
  • Participate in staff development programs, team meetings, and case reviews.
  • Establish and maintain effective working relationships within the team and with supervisors.
  • Complete additional tasks or special projects as assigned by management.
  • Trained and authorized to access protected health information.

Requirements

WHAT YOU NEED:

Education & Experience: Bachelor's degree in human services, 2 year experience in foster care and adoptions or related job preferred.

Physical Requirements: Ability to travel regionally on a regular basis and statewide as needed; ability to bend, lift, and carry 30 pounds.

All Family Care Managers must be able to show proficiency at the conclusion of a six-month probationary period to include:

  • Successful completion of car seat training.
  • Successful completion of pre-service training.
  • Being independently able to represent and discuss case progress in court.
  • Being independently able to complete an accurate and comprehensive face to face home visit note.
  • Have solid and timely documentation in the FSFN system including effective navigation of the system.
  • Have detailed and timely court documents.
  • Be consistently meeting time frames for child visits, parent contact, and FFA/progress updates.
  • Understand community resources and the referral process.
  • Demonstrate excellent customer service by responding within 24 hours to emails and voicemail messages.
  • Have established personal and effective time management processes to include lists, calendars, and other tracking mechanisms.
