The Hospice Services and Volunteer Coordinator LPN is responsible for managing all aspects of the patient intake process, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and assists with managing the insurance verification and authorization processes. Position is also responsible for planning, coordinating, and managing all volunteer program activities in the organization.
Key Responsibilities:

Manage all aspects of the hospice intake process, ensuring seamless transitions and patient-centered care.

Educate patients and families on hospice services and coordinate plans of care.

Establish and maintain strong relationships with referral sources and community partners.

Maintain up-to-date knowledge of regulatory requirements and contract provisions.

Assist with insurance verifications, authorizations, and inter-departmental training for non-clinical functions.

Monitor and report referral trends and contribute to strategic planning and performance improvement efforts.

Develop, maintain, and conduct volunteer training and orientation.

Recruit and select volunteers during multiple recruitment efforts each year.

Match volunteer services to patient and family needs in collaboration with the care team.

Supervise and evaluate volunteer performance and maintain personnel files and monthly statistics.

Provide outreach and education about hospice volunteer services to the community.

Participate in budget planning and quality improvement activities.
Qualifications:

Licensed Practical Nurse (LPN) in Kansas and Missouri (required).

3–5 years of experience in a healthcare setting; home care and hospice experience preferred.

College degree in human services or a related field preferred.

Strong organizational, communication, and interpersonal skills.

Ability to supervise, coordinate, and evaluate volunteer efforts.

Knowledge of hospice philosophy, community resources, and volunteer management best practices.

Proficient in using office equipment and computer systems.
