What are the responsibilities and job description for the Dementia Program Coordinator position at VNA Health System?
VNA Health System, a leader in providing quality home health and hospice care for over 110 years, is seeking a dedicated and organized Dementia Program Coordinator to oversee and enhance our dementia care program. The ideal candidate will possess strong relationship management skills and a passion for program development within the dementia population. This role requires effective supervision, administrative capabilities, and a commitment to fostering positive outcomes for participants and their caregivers.
Ideal candidate will have at least 2 years’ experience in caring for dementia patients, experience in coordination of healthcare programs including implementation of new programs, care navigation, and/or development/oversight of a healthcare program. Certification in dementia care is preferred.
Responsibilities:
- Lead the coordination, implementation, and continuous improvement of the Dementia Care Program ensuring that program objectives align with the goals for dementia care quality, health equity, and caregiver support.
- Work with the interdisciplinary care team to develop and maintain person-centered care plans for beneficiaries, providing them with 24/7 support services and respite care as needed and provide needed education to care givers.
- Provide effective care coordination with beneficiaries’ primary care providers, their care givers, and community-based partners involved in patient care.
- Implement processes for data collection, analysis, and reporting on program outcomes. Provide regular updates to leadership on program performance, including metrics related to care quality, cost reduction, caregiver support, and health equity.
