Job Title
Nurse Practitioner (NP) - VNS Hospice Care
Provides advanced nursing clinical care to patients in the VNS Hospice Care program consistent with VNS Health philosophy and standards of nursing practice, improving hospice utilization for eligible patients.
Main Responsibilities
- Provides advanced nursing care and treatment to hospice patients through home visits and telephonic consultations, including assessment, diagnosis, prescribing medication, and ordering tests and treatments.
- Conducts in-person pre-admission visits for certification of terminal illness by Hospice Team Physicians and completes hospice admissions when deemed eligible.
- Performs re-certification visits at specified intervals for continued service in the Hospice Program.
- Prescribes medication to select patients under a collaborative practice agreement within the scope of a nurse practitioner's practice.
- Demonstrates documentation of patient care services in accordance with VNS Health standards.
- Collaborates with patients, families, primary care physicians, and other team members to provide assessment and care planning.
- Consults with primary care physicians and specialists as necessary for management and disposition plans.
- Participates in on-call NP coverage rotation after hours and weekends for in-person pre-admission visits and completes admissions without required pre-admission visits.
- Travels to multiple regions visiting patients and families.
Education and Experience
- Master's Degree in Nursing required.
- Minimum of three years clinical nursing experience required. Experience as a Nurse Practitioner preferred.
