This position serves as a liaison between residents, health providers and social service agencies in the community. The Community Health Worker helps facilitate residents' access to health and social services to improve their health outcomes and wellbeing. Developing and / or coordinating on-site health and wellness programs. Engaging residents in on-site health and wellness activities. Supporting residents during / following hospital stays and / or doctor's appointments. Facilitating health education and behavior change strategies with individuals and groups. Communicating and working with community health organizations to assist with health navigation. Assisting residents with completing health-related forms and paperwork. Assisting residents with scheduling health-related appointments.
