What are the responsibilities and job description for the Outreach Worker /Driver License position at Vocational Instruction Project?
Description
• Conduct community outreach to engage individuals into treatment, not limited to phone and internet.
• Schedule individuals for program screening and complete “warm hand-off” to specific programs for completion of intake process.
• Conduct outreach to clients having difficulty maintaining adherence to treatment (Mental Health/Medical/Substance Use Treatments)
• Distribute outreach materials.
• Revise and develop presentations as needed under the guidance of Senior Director.
• Conduct community and provider presentations to promote VIP’s programs and services.
• Provide transportation services to VIP clients to and from courts, rehabs, detoxes and other referral sources.
• Document outreach activities and complete all assigned daily, weekly, & monthly reports.
• Maintain strict confidentiality when handling all client/patient information/interactions.
• Attend all assigned agency and department meetings.
• Maintain good working relationship with all agency staff, client, and visitors.
• Provide telephone coverage for 24-hour Access Center, as scheduled, to manage telephone inquiries and provide program information.
• Other relevant duties as required.
Requirements
• High School Diploma/GED and one (1) year experience working in a social service setting.
• CASAC-T or CRPA preferred.
• Prior experience working in a clinical, mental health, housing or substance use disorder treatment setting preferred.
• Valid NYS Driver’s license with a clean driving record.
• Must demonstrate appropriate written, verbal, interpersonal and organizational skills.
• Bilingual English/Spanish a plus
• Ability to interact effectively will all populations served
• Must be computer literate including proficiency with electronic health records, Microsoft Word, Outlook and PowerPoint).
• Must be able to work a flexible schedule including occasional evenings and weekends.
