We are currently seeking a full-time Personal Assistance Service (PAS) Scheduler to provide support within our service area and work closely our Health Maintenance Activity (HMA) pod, which focuses on specialized care for consumers.
As a PAS Scheduler with VFI, you will play a critical role in supporting both consumers and direct care workers, while collaborating with our nursing team to ensure seamless care delivery. Your responsibilities will include:

As a PAS Scheduler with VFI, you will play a critical role in supporting both consumers and direct care workers, while collaborating with our nursing team to ensure seamless care delivery. Your responsibilities will include:
- Scheduling direct care workers based on the specific needs of specialized care consumers
- Frequent in-person meetings with consumers to assess needs and ensure satisfaction with services
- Maintaining accurate documentation of communications with consumers, direct care workers, and nursing staff
- Collaborating closely with nurses within the HMA pod to coordinate care plans for specialized care consumers
- Ensuring billing requirements are met in a timely manner
- Traveling throughout VFI's service area (company vehicle provided)
- Handling on-call work (weekends/evenings) on a rotating basis, as required
- Supporting HMA pod activities, integrating health maintenance goals for specialized care consumers, and coordinating services with the nursing team for optimal outcomes

- Ability to complete all required documentation accurately and promptly

- Bachelor's degree in human services or a related field
- Personal experience within the disability community
- Experience with Home and Community-Based Services
- Familiarity with specialized care coordination and working in a pod-based team structure with nursing staff
