Volunteers of America Delaware Valley is seeking a dedicated Aftercare Treatment Specialist to support individuals as they transition from structured supervision to independent community living. This role plays a vital part in maintaining treatment continuity, ensuring post-release stability, and addressing the long-term needs of justice-involved individuals.
As a member of the treatment team, you’ll assess risks and needs, develop individualized service plans, provide counseling, and support clients through every phase of their aftercare journey.

Volunteers of America Delaware Valley is seeking a dedicated Aftercare Treatment Specialist to support individuals as they transition from structured supervision to independent community living. This role plays a vital part in maintaining treatment continuity, ensuring post-release stability, and addressing the long-term needs of justice-involved individuals.
As a member of the treatment team, you’ll assess risks and needs, develop individualized service plans, provide counseling, and support clients through every phase of their aftercare journey.
What You’ll Do
- Conduct client assessments and create recovery-oriented service plans
- Facilitate one-on-one case management sessions and group workshops
- Monitor client progress, document outcomes, and revise treatment plans as needed
- Provide linkage to community resources such as housing, employment, and mental health services
- Attend court and parole hearings when required and maintain compliance with funder reporting standards
- Facilitate core curriculum programming and assist with house meetings
- Coordinate discharge planning and long-term support strategies
- Participate in treatment team meetings and staff development trainings
- Conduct drug and alcohol screenings and maintain confidential case documentation
- Provide support to program operations and assist with data and reporting
- Bachelor’s degree, preferably in a Human Services field
- At least 2 years of professional experience in mental health counseling or working with special needs populations
- Knowledge of addictive behaviors, group dynamics, and human behavior
- Strong written and verbal communication skills
- Basic computer skills
- Core curriculum facilitation competency or willingness to certify
- Valid driver’s license and ability to meet agency driving standards
- Ability to work flexible hours, including some evenings and weekends
- Must be able to pass a criminal background clearance
- LSI-R training and certification (must be obtained within 90 days of hire)
