What are the responsibilities and job description for the Substance Use Specialist - Camden position at Volunteers of America Delaware Valley?
Responsible for conducting risk/needs assessments and providing the referral to the appropriate level of substance abuse treatment. Responsible for the verification of services and follow-up regarding the necessary interventions and successful engagement.
Scope Of Duties:
- Assessment of all new clients via the approved agency Assessment Tools to determine counseling needs and appropriate level of care.
- Develop Service Plans for all assigned clients based on approved agency assessment tools that are client-centered and recovery-oriented with client input.
- Obtain previous records that are relevant to current treatment recommendations.
- Monitor and document client progress according to funding source standards and/or operational policy and procedures.
- Review, reassess and revise service plan with client input to address ongoing problems and to determine the need for continued services or discharge.
- Schedule and facilitate individual sessions with clients as required.
- Identify and/or develop resources and link clients to needed services.
- When necessary, provide active case consultation and coordination with client-linked resources and referral sources.
- Facilitate scheduled workshops/programs/groups addressing clients' needs.
- Fulfill obligations of training agreements entered into for required treatment interventions.
- Develop appropriate discharge plans for all assigned clients as per contractual requirements.
- Participate in required internal/external Treatment Team meetings.
- Complete all required reports and other documentation.
- Demonstrate competency in program-related treatment modality.
- Interact and communicate (verbally and non-verbally) with all clients, staff, and outside contacts with respect and courtesy.
- Maintain client and staff confidentiality in compliance with administrative policies and procedures.
- Attend and participate in regularly scheduled staff meetings, supervisory conferences, and training sessions.
