POSITION TITLE: Targeted Case Manager
LOCATION : Hopkinsville, KY
STATUS : Full-Time, Salary, Exempt
PROGRAM : Family Recovery Program
REPORTS TO : Clinical Coordinator
JOB SUMMARY:
The Targeted Case Manager provides case management services to families involved in the Family Recovery Program. The primary duty is to assist individuals and/or families with obtaining needed access to housing, medical, social, educational, and other identified services.
KEY RESPONSIBILITIES:
- Provide case management services for families targeted on identified needs of the individual and/or family.
- Conduct comprehensive assessments and reassessments to determine the need for housing, medical, social, educational, and other identified services.
- Develop an individualized case plan for each individual and/or family based on the assessment.
- Refers individuals and/or families to services addressing areas identified on the assessment.
- Monitor and follow-up on goals and activities established on the case plan.
- Identify community resources and develop cooperative relationships with other social service providers.
- Maintain accurate case records and ensure documentation complies with KY Medicaid and accreditation standards.
- Participate in weekly case conferencing, bi-monthly supervision, and monthly staff meetings.
- Assist with special projects and reporting requirements as assigned.
QUALIFICATIONS:
- Bachelor's degree in a behavioral science (including psychology, sociology, social work, family studies, human services, counseling, or another approved human service degree).
- A minimum of one year of full-time employment working directly with adolescents or adults in a human service setting.
- Knowledge and skills in areas of case management, crisis counseling, community resources, advocacy, problem solving, critical thinking, communication, mental health, substance use, and documentation.
