Overview
- Partner with Outreach Staff in offering culturally appropriate dementia education classes to partner organization staff.
- Partner with Outreach Staff to offer exercise and educational classes for participants within partner organizations.
- Provide culturally appropriate memory screenings with group outreach sessions and individual home visits.
Responsibilities
- Support the needs of participants facing dementia and their caregivers.
- Provide group education through Zoom, Webinars, and Workshops.
- Accompany and support project physicians within mobile health screening and dementia diagnostic assessments.
- Partner with project Social Worker in offering culturally appropriate caregiver support groups and evidence base caregiver education series.
Requirements
- Provide care coordination services for assigned project participants.
- Follow-up with participants vial online calls, home visits, and visits to other settings to meet the participants.
- Act as a participant advocate and liaison between participants/family and partner organizations.
- Record participant records.
