About the job:
The Plus Case Coordinator at VOA High School will assist with problem-solving student participation/attendance, behavioral issues and assist with coordination of other supportive mental/chemical health referrals services. The Case Coordinator will meet regularly with clients to discuss progress and case plans and will be responsible for regular data entry. Will transport youth as needed.
Essentials:
- On-Going Professional Development:
- Attends on-going staff development opportunities. Seeks out other learning opportunities to continue to refine practice.
- Participates in school staff meetings and student conferences.
- Data Practices:
- Supports, tracks and assists with problem solving issues associated with students participating/referred to the ALC Plus Program.
- Collects and accurately interprets, utilizes and reports student data to inform on-going support practices. Prepares reports or shares data as appropriate to other staff at the school site.
- Assures program participant privacy is maintained by being knowledgeable and following HIPPA/FERPA data privacy and security regulations.
- Keeps necessary case notes documenting contact with program participant, family and collateral contacts.
- Maintains appropriate paperwork for each enrolled Plus participant.
- Collaboration:
- Consults and collaborates effectively with parents/guardians, educational staff, administrators and other educational/community resources regarding students with identified concerns and issues.
- Transports young people to school (if appropriate) and to appointments as need ideally outside of school hours.
- Works closely with the VOA Academic and Student Services staff to coordinate academic progress, goal support, and career planning.
- Crisis Management:
- Provides advice, guidance and support to students, parents/guardians in times of crisis that is trauma informed and culturally competent.
- Assists and supports students who are experiencing emotional, behavioral, or health related issues and/or students who have difficulty attending school on a consistent basis. Identifies referrals and ensures transition into appropriate services.
- Introduces and refer students to resources that may assist them with personal and educational issues.
- Develops relationships with external organizations that provide needed supports for ALC Plus participants.
- Goal Setting & Coaching:
- Works with students to develop goals around academic performance, behavioral and attendance and assists with appropriate interventions in conjunction with the other ALC Plus team members.
- Promotes positive social skills and appropriate pro-social behavior with referred students.
- Participates in the integrated/multi-disciplinary intake team to assist in the development of a complete Student Service Plan that includes goals.
- College and Career Planning:
- Collaborates with school staff to ensure each young person has a postsecondary and career goal. Regularly checks in to ensure progress toward that goal.
- Culturally Competent Practices:
- Communicates and develops relationships that demonstrate deep understanding of the cultural heritage of each student and their family/caregivers.
- Participates in recreational and educational after school activities that have meaning to students.
- Travels to settings where services are provided, primarily family, home, and school and community resources.
- Provides an appropriate professional role model for families and encourage/support parents/caregivers in the parenting of their children.
- Trauma Informed Care:
- Uses trauma informed responses to young people and their families.
- Practices “compassionate accountability” in helping students reach their goals.

Required Qualifications:
- Associates degree in Human Services or related field required
- Bachelors degree preferred
- Two years of experience in working with adolescents and families
- Knowledge of Education and County Probation and Human Services Systems
- Knowledge of case management services

The VOA High School education team is small, goal-oriented, highly collaborative and committed to nurturing student voice within the educational experience. VOAHS is part of Volunteers of America MN/WI which serves as an affiliate of the Volunteers of America parent organization.
The Plus program started in 2010 to support youth who are county involved and experiencing homelessness to better educational outcomes and ultimately to careers with family-sustaining wages. The Case Coordinator will be responsible for the day-to-day oversight and management of assisting enrolled Plus students to achieve success in reaching personal, educational and career goals. The Case Coordinator will function as part of the school site. The Case Coordinator will consult with the school's educational staff, Plus Manager, Plus Supervisor, Hennepin County Workers (Probation and Court Staff, Child Welfare, Truancy Workers) and parents/caregivers to assure participant success in the program.
