Shift Details: Full time - 40 hours/week
Schedule: 8:00 AM - 4:00 PM (approximately) with occasional extended time, as needed
Compensation: $75,000-80,000 annual salary
Location: 22426 St. Francis Boulevard, St. Francis, MN 55303
About the job: Unique opportunity providing service to adolescents (and their families) with mental/chemical illness. Responsible for assessing resident's needs and developing a plan of care, implementing the care plan and adjusting the plan as needed.
Essentials:
- Assess resident's nursing needs
- Develop resident's plan of care
- Provide assessed nursing care needs to resident
- Monitor resident's progress
- Provide needed assessments and examinations
- Administer medications prescribed by physician
- Train others in medication administration as prescribed by physician
- Perform any needed medical procedures such as dressing wounds, etc.
- Perform any needed emergency procedures for residents
- Coordinate resident care with care team, outside providers and agencies, family members
- Provide health education including self-care and wellness to patient and their families
- Provide other necessary training for staff and clinicians
- Serve as advocate for residents
- Supervise the current health and medical record of each resident
- Maintain current MN registration and CPR/First Aid training and maintain required CEU credits
- Will complete all tasks in alignment with and according to the MN State Board of Nursing
- Follow agency policy and remain compliant with DHS licensing standards
- Inform appropriate management of pertinent medical findings and recommendations for follow-up with outside agencies
- Schedule routine physicals and medical/dental examinations as needed
- Promote personal safety and a safe environment for participants and coworkers
- Responsible for ordering needed medical equipment and supplies
- May plan and coordinate community programs to promote health
- Assure that program participant privacy is maintained by being knowledgeable and following HIPAA privacy and security regulations
- Work as many hours as needed to complete the job
- Must maintain a valid MN Driver's license
- Will participate in the on-call rotation
