CLIENT SUPPORT & SERVICE COORDINATOR

Position Type: This is a Full-time, in-office position with occasional work from home opportunity and requires a 40-hour week on average. May include occasional weekend and/or evening hours as assigned.
Environment: Physical demands include sedentary work, operating a computer and other office productivity machinery with frequent communication with clients and colleagues of VOA Illinois and others involved in the care and custody of clients. The individual hired must be able to move, traverse and travel in the community in a vehicle, assist clients in and out of a vehicle if necessary and transport clients in the community.

Program Overview: Support Services for Veteran Families (SSVF) is a comprehensive program for Veterans who are struggling in crisis or at risk of serious instability in their lives. The program provides Veterans with the support, resources and hope they need to successfully win “the war at home” and achieve their full potential in all areas of their life, by working on a personalized service plan with our Veteran Services team. Our team offers a full continuum of supportive services and interventions that are individually tailored to meet the unique needs of each Veteran.
Essential Functions: The Client Support & Service Coordinator is responsible for providing client-centered, relationship-driven and family-focused services and interventions to individuals or families served by VOA IL. The development of contacts with service providers and agencies for resident referrals; educating clients on available services and monitoring the provision of services by VOA IL.
- Conducts general case management, including evaluating clients’ psychological and physical needs and developing service plans when not provided by the community.
- Meets with program participants to offer support and monitor progress on service plans.
- Delivers culturally and socioeconomically sensitive services.
- Serves as a liaison between community agencies, service providers and clients.
- Utilizes the least intrusive intervention required to address problematic situations.
- Facilitates and monitors for the delivery of appropriate, timely and satisfactorily services to clients.
- Educates clients, both individually and in groups on service availability, application procedures and client rights.
- Advocates for and negotiates on behalf of clients to ensure the timely and cost-effective provision of services.
- Meets with service providers as necessary and appropriate.
- Explores opportunities for additional services from private, local, state and federal sources.
- Performs data entry using databases such as Service Point and HMIS.
- Upholds the mission and values of VOA Illinois at all times.

Qualifications
Qualifications:
- Experience in service management, including organizing, problem-solving and advocating.
- Experience and demonstrated proficiency/success in leveraging and working with community resources.
- Ability to communicate effectively and maintain good relations with clients and employees.
- Excellent communication, writing, problem-solving and organization skills.
- Individual must have a valid driver’s license, access to a safe and reliable vehicle during all work hours and have adequate and current vehicle insurance.
- Proficient in Microsoft Office, especially spreadsheets and database and reporting tools.
Education and Experience:
- Bachelor’s degree from an accredited college or university required. Acceptable degrees include those related to human services, social work, psychology, counseling, etc.
- Minimum of one year of case management experience required.
- Must be proficient in MS Office, specifically Word, Excel, PPT and Outlook.
Compensation and Benefits: Base Salary Range: $46,000 - $55,000
